Mail packet — NJ PAAD application

1. Review the filled NJSave application on the next page. The member's drawn signature is in Section D.
2. Keep a copy of the signed application for Margaret's records.
3. Attach copies of the checklist proofs: proof of NJ residence and income documentation (Social Security award letter, pension statement).
4. Mail everything to the address below. Certified mail is recommended but not required — keep the tracking number with the case.
